I was asked at the reunion and have received emails from non-attendees about a secondary run of shirts.
I will be happy to take orders for anyone who desires additional shirts, or did not get one to begin with.
Please allow 3-4 weeks for delivery.

2. Fill out completely
3. Fax back both to: 818.707.8013
4. Mail order form and check to my address

DO NOT email credit card info - it is unsecured.
I can also bill your Pay-Pal account if you send me your account info.

California residents add 8.25% sales tax
I will add the shipping charges on your final bill.
